Role: Social Prescribing Link Worker – Pendle West PCN
Location: Pendle West, East Lancashire
Hours: 37 hrs/week (flexible)
Salary: £32,148 + 5% pension contribution
Contract: 12 months (maternity cover)

About the role:
As a Social Prescribing Link Worker, you’ll make a real difference to people’s health and wellbeing by connecting them with community activities, services, and support networks. You’ll work with patients experiencing long-term conditions, loneliness, or complex social needs, helping them take control of their health and live independently.
What you’ll do:
  • Provide personalised, person-centred support
  • Build relationships with GP practices and community organisations
  • Connect people to local activities and services
  • Support volunteers and community-led initiatives
  • Collect and report on impact data
You are:
  • Empathetic and person-centred
  • Skilled at building trusting relationships
  • Committed to reducing health inequalities
  • Motivated to empower individuals and communities
Experience & Skills:
  • Working in adult health & social care, community development, or public health
  • Supporting people with mental health or long-term conditions
  • Knowledge of local VCSE services
  • IT literate and able to manage a caseload
