The Social Prescribing for Children & Young People service is a pilot project operating in the Pendle West Primary Care Network (PCN) area only. If you're unsure of which Pendle PCN the referred client belongs to, complete the referral form anyway and if the client belongs to Pendle East PCN the referral will be handled by our Health & Wellbeing for Children & Families team.
What Happens Next?
The referral will be allocated to a Social Prescribing Link Worker or Community Connector. We typically try to allocate referrals to a Link Worker/Community Connector within 48 hours of receipt. The Link Worker/Community Connector will then contact the client directly and discuss their needs and arrange any necessary support.
