Reasons indicated on initial referral

TG was referred from the GP Practice with the following issues:

Social isolation, very few friends and minimal contact with family, mental health and depression, low self-esteem, debt issues, finances, and employment support.

Background of client

TG lost his wife very suddenly and was struggling to cope with running the house, struggling with loss of his wife, neglecting himself, and wanting help to find employment. However he needed help with lots of practical stressors first in the hope that he can then move forward and find employment which is his ultimate goal.  

Initial Assessment and Support Provided

Initially the most important thing to TG was to obtain some photographic ID for a number of reasons: open more suitable bank account, complete and obtain CSCS card.  TG had neglected himself a little since his wife passed away and was in need of new glasses which I supported him with in making appointments at opticians. I have also supported him with Jobcentre appointments securing the funding for the photographic ID and a mobile phone. The ID will enable him to start working with Age of Opportunities.  I referred TG to Lancashire Women for debt advice and accompanied him to appointments at TG's request due to lack of confidence. TG has done really well and is now happy to attend these appointments without any support.  I am continuing to support TG with his mental health, finances and employment/training.
